Nutrition guidelines for twin pregnancy: What changes when you're expecting two
"You are having twins." Four words, and suddenly your baby name list needs a sequel. Whether that ultrasound moment had you laughing, crying, or doing quick math on how many cribs fit in the nursery, one thing follows close behind the surprise: more to learn, a little faster than expected. Carrying twins changes the math on nutrition too. Your body is building two placentas, supporting a larger blood volume, and growing two babies at once. Most official pregnancy nutrition guidelines are written for a singleton pregnancy and then loosely extrapolated to twins. Twins means more calories, more protein, and closer attention to nutrients like iron, folate, choline, vitamin D, calcium, iodine, and omega-3s, but it also means the "just eat more" advice needs real structure behind it, especially once nausea, early fullness, or reflux enter the picture. How many calories do you need when pregnant with twins?
The simple answer: there is no single official number. The reason? Twin pregnancy has rarely been the subject of dedicated nutrition research. What we do have is a consistent theme across the studies that exist: energy needs are meaningfully higher than in a singleton pregnancy, and they climb as the pregnancy progresses.
A frequently cited estimate puts caloric intake for a woman with a ‘normal’ pre-pregnancy BMI (18.5-24.9) at 40 to 45 Kcal per kilogram of body weight per day. In practical terms, that is roughly 2,800 to 3,150 calories a day for a 70 kg (about 154 lb) woman. A 2018 study measuring actual energy requirements across a twin pregnancy found intake needs rising by about 700 Kcal/day in the second and third trimester as compared to the first. A landmark Canadian program that provided structured nutrition counseling and an additional 1,000 calories and 50 grams of protein per day after 20 weeks reduced preterm birth by 30 percent and increased twin birth weight by an average of 80 grams compared to usual care. It’s key to note that, as always, nutritional needs should be individualized dependent on lab work, body weight and mass, and history.
The reality when it comes to consuming those calories, however, is that appetite doesn't always align with needs. Nausea, vomiting, and early satiety are more common in twin pregnancy than in singleton pregnancy, and studies tracking actual intake find that many women fall short of theoretical targets, particularly in the first trimester. This is normal. What tends to help is treating the calorie target as a direction to build toward gradually rather than a number to hit from day one, leaning on calorie-dense, nutrient-dense foods (nut butters, whole-fat dairy, avocado, eggs), and shifting from three large meals to five or six smaller ones to support better meal tolerance.
What are the essential macronutrients for a twin pregnancy?
Protein gets the most attention in twin-specific research, and for good reason: inadequate protein has been linked to impaired fetal and placental growth. Some clinical guidance suggests protein should make up about 20 percent of total calories, with carbohydrates and fat each around 40 percent — a notably different split from typical singleton guidance, largely because of the elevated risk of gestational diabetes in twin pregnancy and increased protein needs for optimal growth. In practice, many women pregnant with twins need somewhere in the neighborhood of 100 to 150 grams of protein a day, best confirmed with your own care team based on your weight and trimester.
Pairing protein for staying power
For nutrient density and steadier energy, the trick is to combine protein with fat and fiber rather than eating it alone. A few combinations that do this well:

What key vitamins and minerals matter most in a twin pregnancy?
There is very little research on the actual vitamin and mineral need numbers in a twin pregnancy. However, the below are some crucial nutrients in pregnancy, which includes that for multiples.
Folate. Adequate folate intake is key to support a reduced risk of neutral tube defects. The standard pregnancy recommendation is 600 mcg per day, with a minimum of 400 mcg expected to come from supplementation. The risk of folate-deficiency anemia is estimated to be about eight times higher in twin pregnancy than in singleton pregnancy. Some maternal-fetal medicine guidance recommends a higher need in twin pregnancy — up to 1,000 mcg per day from food and supplements combined, which is also the tolerable upper limit for adults, so this is a ceiling, not a target to exceed.
Vitamin D. Several studies have found lower vitamin D levels in twin pregnancies compared to singleton pregnancies at the same point in gestation, though a formal, twin-specific intake recommendation hasn't been established.² Given how common vitamin D insufficiency already is in pregnancy generally, this is a nutrient worth monitoring with bloodwork and noting that the evidence indicates 4,000 IU/day for pregnant women is typically safe and effective.
Iron. Iron needs are estimated to be about 1.8 times higher in twin pregnancy, driven by a larger blood volume expansion and higher red blood cell production. Anemia is correspondingly more common, affecting an estimated 30 to 45 percent of women pregnant with twins by the third trimester. Older guidance recommended roughly 30 mg of elemental iron daily starting around week 12, though newer research cautions against supplementing beyond what's needed, since excess iron has been linked to its own risks. Iron needs should be determined individually with your provider and this can include a full iron panel - inclusive of baseline ferritin - rather than assuming more is better.
Calcium. Findings are mixed on whether blood calcium differs meaningfully between twin and singleton pregnancy, but some research points to higher bone turnover in women carrying twins, suggesting calcium is being drawn on more heavily. Recommendations for total daily calcium (food plus supplement) in twin pregnancy have ranged from roughly 2,000 to 2,500 mg, with a practical note that single doses above about 500 mg absorb less efficiently and are more likely to cause GI discomfort. If additional calcium supplementation is warranted, it's typically better spread across the day and taken a couple of hours apart from an iron supplement to optimize absorption.
Iodine. General pregnancy guidance calls for about 150 to 200 mcg per day, but there's no specific twin-pregnancy figure in the literature yet, so as with any supplementation, it is key to speak with your medical team about whether additional supplementation beyond this is warranted for you.
Omega-3s (DHA/EPA). Some clinical guidance for twin pregnancy suggests 600 mg DHA and 120 mg EPA, with some recommendations up to 1,000 mg omega-3 total daily. In singleton pregnancies, omega-3 supplementation has been associated with meaningfully lower rates of early preterm birth and a modest increase in average birth weight, which is part of why it's considered worth prioritizing in a twin pregnancy as well.
Carrying twins doesn't automatically double every nutrient requirement — the honest answer, based on the current evidence, is that needs are higher for several key nutrients but the exact multiplier isn't well established for many of them. That's exactly why diet, prenatal supplementation, lab work, and your individual health history all need to work together, rather than defaulting to a single blanket number. The nutrients care teams tend to track most closely in a twin pregnancy are iron and ferritin, vitamin D, and folate status, simply because those are the ones with the clearest signal of increased demand and potential impact on health outcomes.

Hydration needs also climb and are even greater in a twin pregnancy than in a singleton one.² To meet needs, simply focus on steady water intake throughout the day. Fill up your water bottle first thing in the morning - before coffee - and have a goal to refill it in the early afternoon.
